Registration as Students must be accompanied with a proof of status.
Registration fees paid by credit card will be converted into New Taiwan Dollars, based on a fixed foreign exchange rate of US$ to NT$ which is US$1 to NT$33.

REGISTRATION FEES

Registration fees for overseas participants are in US Dollars and for domestics participants are in New Taiwan Dollars. All participants are reminded that bank charges (if any) are to be paid by registrants, which is over and above the final registration a fee.
To qualify for special rates for Students, a proof of applicant status is required (e.g. Student Card).
To attend either one of the pre-congress meetings on Nov. 29, 2008 (4th Annual Meeting of GCACMID & Pre-Congress Meeting of the 5th Asian UTI/STD Forum), please register for full-program registration.

REGISTRATION METHOD
Participants are strongly requested to register for the congress and social programs individually via On-line Congress System at the congress website. Those who have difficulty in processing via the internet, to fax or mail the registration form along with the payment to the Congress Secretariat is also accepted no later than the deadline for pre-registration date, Sep. 15, 2008. (The deadline for pre-registration is extend to Sep. 28, 2008)

CONFIRMATION OF REGISTRATION
A confirmation of registration will be sent to you automatically via e-mail upon receipt of your completed registration form and the full payment.

REGISTRATION DEADLINE
The deadline for pre-registration is September 28, 2008. On-line registration system will be closed then.
All registration and payment received after this date will be considered as On-site Registration and processed on-site.

PAYMENT METHODS
Payment must be made in US Dollars or NT Dollars and the payment could be made in the either of the methods mentioned below. Please note that participants will take care of all bank charges incurred.

Payment for On-site Registration

LETTER OF INVITATION
Participants may download the invitation letter from the congress website directly. The invitation is issued only for the purpose of entry visa application and is not a commitment from the 11th WPCCID organizers to provide financial aid to any applicant or their accompanying persons. Expenses incurred are the personal responsibility of the applicant.

CANCELLATION AND REFUND POLICY
Notification of cancellation and request for refunds must be sent in writing to the 11th WPCCID Congress Secretariat. The following cancellation and refund policies will apply:
Before Sep. 15, 2008: Full refund of prepaid fees except for an administration charge of US$50.
Sep. 15 to Oct. 15, 2008: 50% of Registration Fee
After Oct. 15, 2008, no refund is applicable.
All refund will be processed after the congress has concluded.

DISCLAIMER
Every attempt will be made to present the program as printed. However, the 11th WPCCID reserve the right to alter or cancel, without prior notice, any of the arrangements, programs or other items relating directly or indirectly to the Congress, for any cause beyond its reasonable control. The 11th WPCCID Congress Secretariat is not liable for any loss or inconvenience caused as a result of such alteration. In the event of cancellation of the Congress all pre-paid fees will be refunded in full. However, the 11th WPCCID Congress Secretariat is not liable for any loss or inconvenience caused as a result of such cancellation.

INSURANCE
Participants are advised to take care of their own travel and medical insurance and to extend their policy to cover personal possessions as the Congress does not cover individuals against cancellation of bookings or theft or damage to their belongings.
